# Resident Health Records

The Health Records tab (which can be seen when viewing a resident from the ***RESIDENTS*** screen) lists all the different types of records that need to be filled in when a resident has a health issue. \
\
These include wounds, infections, pressure ulcers and moisture lesions etc.

### Adding Health Records

To add a new record click the blue ***+ ADD*** button after selecting a record type. This will then open the form that allows you to enter the relevant details (see below)

On the form, you will need to answer ALL the required questions. Note there may be multiple pages and you can navigate through each page using the ***PAGES*** bar, or by clicking the ***PREVIOUS*** and ***NEXT*** buttons.

Once you have entered all the required info (and any relevant optional info) you will be able to click the green ***SUBMIT*** button to record it. If the green button is not available to you, this is because something required is missing.

### Viewing Health Records

When you have added a health record, it will appear in the list for the residents details under the ***HEALTH RECORDS*** tab.

You will see the current status (which is either ***OPEN*** or ***CLOSED***)

#### - Open records

Denotes that this is still a problem for the resident. Keep it open until the issue is fully cleared. To close a record, just click the red ***CLOSE*** button. It will ask you to confirm that this health incident is now fully cleared. You will also be able to view, edit and delete the record (if you have the relevant permissions to do so).

#### - Closed records

Shows a past health issue for a resident that is now fully cleared. If it has been closed by mistake, you can reopen it by hitting the green ***REOPEN*** button.

## Querying This Documentation
If you need additional information that is not directly available in this page, you can query the documentation dynamically by asking a question.

Perform an HTTP GET request on the current page URL with the `ask` query parameter, and the optional `goal` query parameter:

```
GET https://guide.guardiancaregov.com/documentation/residents-menu/integrations.md?ask=<question>&goal=<endgoal>
```

`ask` is the immediate question: it should be specific, self-contained, and written in natural language.
`goal` is optional and describes the broader end goal you are ultimately trying to accomplish on behalf of the user. GitBook uses it to tailor the answer towards what is most useful for that goal.

The response will contain a direct answer to the question and relevant excerpts and sources from the documentation.

Use this mechanism when the answer is not explicitly present in the current page, you need clarification or additional context, or you want to retrieve related documentation sections.
